Transparency, Professional Boundaries, and Regulatory Compliance

CoreMD Insights and CoreMD Clinic (collectively, “CoreMD”) are committed to maintaining the highest standards of professional integrity, regulatory compliance, and transparency in all aspects of their operations.

This statement outlines the structural, ethical, and operational safeguards implemented to ensure clear separation between CoreMD services and any other clinical environments in which the physician may practice, including, without limitation, the Albany Medical Clinic.

1. Independent Legal and Operational Structure

CoreMD operates as an independent entity, separate and distinct from any hospital, clinic, or healthcare institution where the physician may hold privileges or provide medical services.

Without limitation:

  • CoreMD maintains its own infrastructure, systems, and workflows

  • No shared electronic medical record (EMR) systems are utilized

  • No integration exists with institutional databases or patient records

  • Billing, administration, and service delivery are conducted independently

Under no circumstances does CoreMD access, rely upon, or utilize any proprietary or institutional resources belonging to third-party clinical organizations.

2. Absence of Patient Solicitation or Targeted Recruitment

CoreMD does not engage in the solicitation, targeting, or recruitment of patients from any external clinical practice or institution.

Specifically:

  • No direct or indirect communication is initiated toward patients of external clinics for the purpose of enrollment

  • No use is made of institutional patient lists, records, or contact information

  • Participation in CoreMD services occurs solely on a voluntary, self-initiated basis

Patients may access CoreMD exclusively through publicly available channels, including its website and independent communication platforms.

3. Separation from Publicly Funded Healthcare Services

CoreMD services are structured to operate independently from publicly funded healthcare systems, including those governed by OHIP.

Accordingly:

  • No insured medical services are duplicated or replaced

  • CoreMD services do not constitute a prerequisite for access to publicly funded care

  • Participation in CoreMD has no bearing on a patient’s entitlement to, or access to, care within any other healthcare setting

CoreMD is designed to function as a supplementary, non-insured service offering.

4. Strict Separation of Clinical Contexts and Data

CoreMD maintains strict separation between:

  • Medical services rendered within external clinical environments; and

  • Services provided through CoreMD

This separation includes, without limitation:

  • Distinct documentation systems and medical records

  • Separate communication platforms and channels

  • Independent data storage and processing environments

No transfer, duplication, or use of patient information occurs across these contexts except where expressly authorized by the patient and conducted in accordance with applicable privacy legislation.

5. Voluntary Participation and Protection of Patient Autonomy

All participation in CoreMD services is entirely voluntary.

Patients:

  • Are under no obligation to enroll in CoreMD

  • May discontinue services at their sole discretion at any time

  • Will not experience any impact, limitation, or preferential treatment in any other clinical setting based on their decision to participate or not participate

CoreMD does not exert influence, directly or indirectly, over a patient’s clinical decisions within other healthcare environments.

6. Compliance with Professional and Regulatory Standards

CoreMD is structured and operated in accordance with the professional, ethical, and regulatory standards established by the College of Physicians and Surgeons of Ontario, as well as all applicable federal and provincial laws.

This includes, without limitation:

  • Avoidance and mitigation of conflicts of interest

  • Clear delineation between insured and uninsured services

  • Protection of patient autonomy and informed consent

  • Compliance with privacy and data protection legislation

All services are designed and delivered in a manner consistent with accepted standards of medical professionalism and ethical conduct.

7. Scope and Limitations of CoreMD Services

CoreMD provides structured health insights, longitudinal tracking tools, and educational support.

CoreMD does not:

  • Replace primary care services

  • Provide emergency medical care

  • Assume responsibility for comprehensive medical management outside its defined scope

Patients are advised to maintain an ongoing relationship with their primary healthcare provider for all medical needs outside the CoreMD service framework.

8. Commitment to Transparency and Institutional Respect

CoreMD acknowledges and respects the role of established healthcare institutions and is committed to operating in a manner that does not interfere with, compete improperly with, or derive undue advantage from such institutions.

Any physician affiliated with both CoreMD and an external clinical organization shall ensure that all professional activities are conducted with full transparency, appropriate separation, and adherence to applicable ethical obligations.

9. Inquiries and Institutional Safeguards

In addition, CoreMD expressly affirms its commitment to maintaining full compliance with all contractual, ethical, and professional obligations applicable to the physician, including those arising from existing professional associations with institutions such as the Albany Medical Clinic.

Without limitation, CoreMD has been structured and is operated in a manner specifically designed to avoid any actual, perceived, or potential conflict of interest, including:

  • No solicitation of patients from any external clinic, including Albany, whether directly or indirectly

  • No use of patient lists, medical records, or institutional data, in accordance with privacy obligations and PHIPA requirements

  • No redirection or interference with existing patient relationships established within Albany or any other clinical setting

  • No billing of patients for services rendered within Albany, and no duplication of services covered under publicly funded healthcare systems

  • No use of Albany infrastructure, systems, staff, or resources, including EMR platforms, administrative processes, or physical facilities

Furthermore:

  • Any patient who elects to use CoreMD services does so entirely on their own initiative, through publicly accessible channels

  • No discussion, promotion, or enrollment into CoreMD services occurs within the context of clinical encounters at Albany

  • All services provided through CoreMD are delivered in a fully separate legal, operational, and clinical environment

CoreMD further acknowledges and fully respects:

  • All non-solicitation obligations applicable to the physician

  • All confidentiality obligations relating to institutional information and patient data

  • The independent nature of patient choice in determining where to seek care, without inducement or influence

CoreMD is committed to ensuring that its activities:

  • Do not interfere with, compete improperly with, or undermine any clinical institution

  • Do not create any diversion of patients, resources, or revenue from such institutions

  • Remain fully aligned with the standards set by the College of Physicians and Surgeons of Ontario and applicable contractual frameworks
